Each Dragon will start with a pearl that will be moving alongside the body of the dragon and you will be damaged whenever the body piece with the Red Pearl reaches the head. You can strategically use the clouds to pass through them, and placing the red pearl on top of the cloud avoiding damage for that period of time.
