In Four Horsemen, each player takes the role of an immortal hero that is trying to prevent the end of the world. To save mankind, the heroes must collect powerful relics, combat demonic hordes, and delay the final ride of the four horsemen. Four Horsemen features a unique deck-upgrading mechanic. Players acquire relics that improve their deck while sacrificing other cards to combat apocalyptic events. Facing the hordes, players lose divinity and gain corruption, potentially causing them to become fallen angels. To win, players need to defeat the apocalyptic mastermind, prevent all four horsemen from reaching their destination, and save humanity from the demonic invasion. Gameplay consists of an event round, an action phase, and a cleanup phase. During the event phase, new demons are added to the map, the horsemen move, and player attributes drain. During the action phase, each hero selects four relics from their hand. These cards have a variety of effects depending on how they are used. One of the chosen cards is scarified, removing the card from the game. During the cleanup phase, heroes acquire new cards, take damage, and prepare for the next round.
